Moodle updates

Included in this emergency maintenance are small fixes or updates to Moodle that have been identified at the start of the semester. * Course banner default backgrounds for custom themes. * Download all submissions (assignment) error resolved. * Choice group allow activity update if no previous groups exist. * Add back the new window icon
